Application Form : The application form , along with necessary instructions, an acknowledgement card and envelope for sending application is obtainable from the office of Haryana Public Service Commission, Bays No.1-10 Block-B, Sector -4 Pancukula by making a payment at following rates:

The cost of Application form will be Rs 1050 For General category 300/ for Reserved category of Haryana if obtained by post . In this case the fee shall be required to remit through Indian Postal Orders or Bank Drafts Payable to the Secretary, Haryana Public Service Commission at Panchkula on or before 27/12/2011, Application can be purchased on cash payment at the counter from 10.00 AM to 1.00 PM and 2.00 P.M. to 4.00 P.M on all working days.

Submission of Application Form : Candidates should ensure that the duly filled application forms for admission to the Preliminary Examination must reach Commission's office at Bays No 1-10 , Block-B, Sector -4, panchukula by post or by personal delivery by close of office working on or before 27/12/2011
